Please use the form below to request scheduled time off.

This form is NOT for sick days. Please email hello@orrettmusicacademy.com ASAP for last minute absences (under 48 hours).

Your request will be reviewed by a member of our admin team in 1-3 business days. If you need to know by a specific date please document that in the appropriate section on the form.

Once we have received and reviewed your request we may contact you to discuss options for either finding a supply teacher to cover the lessons AND/OR for your lessons to be rescheduled by you (the teacher) for another day.

Time Off Request

Each request is for a single day OR consecutive days. Please add separate requests by using the "Add Another Request" button below.

As per your contract:

Teacher Absences      

  • Future Absences (more than 48 hours’ notice): THE TEACHER must complete a time off request form and follow up with an e-mail to the Academy Manager or Administrative Assistant directly for any upcoming absences, giving as much notice as possible, and provide a reason for the absence.
  • Last Minute Absences (less than 48 hours’ notice): Last minute absences are for illness, injury, or other urgent matters only. It is required that THE TEACHER give as much notice as possible. THE TEACHER must text/call the Academy Manager, Administrative Assistant, or another member of the administrative team as prescribed by ORRETT directly. If unable to reach them, THE TEACHER must call the school directly. THE TEACHER must remain available by phone/text until they receive a response from a member of the administrative team to confirm receipt of notification of their absence.
  • Notification to Parents: With both types of absences, under most circumstances, THE TEACHER is not to let the students/parents know THE TEACHER will be away. ORRETT administrative staff will notify students/parents of THE TEACHER’S absence and set them up with a supply teacher, online lessons, or inform them of cancellations as needed.
  • Expected Student Absences: THE TEACHER is required to inform the ORRETT administrative team whether any students are expected to be away the day(s) THE TEACHER is to be absent. THE TEACHER must keep their reschedule tab up-to-date for this reason.
  • Teaching Online: THE TEACHER is required to inform ORRETT administrative staff of any students anticipated to be missing the day(s) they are away, and whether THE TEACHER will be able to teach any students online that have a status of “Hybrid” on those day(s). This does NOT automatically mean that THE TEACHER will be teaching online, as ORRETT policy is, as often as possible, to provide an in-person supply teacher.
  • Canceled/Rescheduled Lessons: For lessons that cannot be provided (either by a supply teacher or online by THE TEACHER) on the day(s) that THE TEACHER is away, THE TEACHER is responsible for offering make-up lessons at a future date as soon as possible. Unlike lessons missed by the student, lessons missed by THE TEACHER that are not made up to the student are refundable to the student and are not paid to THE TEACHER.
Contract Acknowledgement: TEACHER ATTENDANCE "THE TEACHER shall be available for student lessons on their scheduled teaching days on a regular basis. If THE TEACHER misses more than three (3) shifts on any particular day of the week (between September and June), then a meeting will be required with the MANAGER or ASSISTANT MANAGER to determine a course of action, which may include contract termination."
